Please print out the form and fill out the form using a pen. Please include the school name or person’s name to whom you are sending the transcript as well as the mailing address.
For an Official Transcript:
Pay $5 to purchase your transcript. PayPal will accept credit, debit, Venmo & PayPal.
For an Unofficial Transcript:
Please write unofficial on the form and then complete the form. Unofficial transcripts are free.
Once the form is completed, please scan the form and include as an attachment. Send it with the subject “Transcript Request” to admissions@asumidsouth.edu. You may also fax it to (870) 733-6710. Once we receive the form, we will send out your transcript to your desired receiver within 72 working hours. You will be notified if any holds are preventing the transcript(s) from being mailed.
